1. Complete our secure, HIPAA compliant intake form by clicking “Request an Appointment” below

   • Click the button below to access our HIPAA-compliant form.

2. We’ll review your information

   • Our team will verify your insurance and contact you within 1 business day to confirm details and ask if you’d like to proceed with care.

3. Account Setup

   • We will set up a patient account for you in our system, Osmind. This is our patient scheduling system, where you will receive prompts to download the app, complete intake and consent forms, and sign necessary agreements.

4. Schedule your first appointment

   • Choose a time that works best for you either in your newly set up Osmind portal or by calling us — virtually or in person (where available).
